ATTENTION! SYMPHONIC AND CONCERT BAND REHEARSAL DATES SWITCHED

Concert Band and Symphonic Band have switched after school rehearsal dates for this week. 

SYMPHONIC BAND will meet Tuesday, May 12 from 4-5pm and

CONCERT BAND will meet Thursday, May 14, from 4-5pm.

ASBURY LEADERSHIP WORKSHOP

Lafayette Band students have the opportunity to participate in a special workshop on Wednesday, June 3, at Asbury University. Mr. Paul Stephens, Director of the Educational Leadership Department at Asbury, has designed a one-day event especially for our students. Joining Paul will be Kara Mattingly, Director of the Asbury Challenge Course. Kara will focus on team building skills, peer interactions and modeling behaviors for our students. This event will also include completion of the high ropes course! 

On Wednesday, June 3, students will report at 8am to the Lafayette band room. Round trip transportation and lunch will be provided and the day will conclude at 5 pm. There is a cost of $40 per student to cover expenses. Register & pay on the website atwww.lafayetteband.org .

Please make the checks payable to LBA and send to: Lafayette Band Association, Accounts Receivable, PO Box 910396, Lexington, KY 40591. The waiver form (waiver /medical) must be completed and returned to Mr. Smith by Tuesday, May 26. The form can be found on our website at www.lafayetteband.org.

 

MEDICAL FORMS

Thank you to all that have turned in medical forms. It is critical not to mention mandatory that we have at least the first page of the medical form and all its information on file. If you missed turning in your child’s medical form then bring them to the parent meeting Monday night.  

Remember we cannot give your child even an antacid without a physician’s signature and your approval on the second page of the form. It is perfectly acceptable if you prefer to wait and have your physician sign the second page of the medical form when your child receives their physical.  The second page is not valid until a physician signs the form.  

Turn in the medical form at the May 11 LBA Parent Meeting or if you cannot make the meeting mail the form to: Mike Booth 1105 Foleys Retreat 40514.

 

STUDENT ANNUAL PHYSICAL AND KHSAA FORM

Make an appointment now with your child’s physician to get a yearly physical. Have them complete and sign the KHSAA Physical Form and send it with you student on the first day of band camp. It is a good idea to take a blank KSHAA Form with you since not all physicians keep blank forms on file.

If your student had a physical last year after July just make arrangements for a new physical as soon as possible at the one year date.
